It’s killing him inside that she has to go through this humiliation & that he has to watch Alfonso all over her (hence the near tears). He’s also trying to bring a semblance of a comforting presence to her (hence the not blinking). He’s not mouthing anything discernible to her but my take on it that he knows that she’s seeking him out with her eyes & he’s picturing himself in Alfonso’s place (hence why his mouth drops about at the same time every time she cries out). Cesare takes a long time to gather himself and get up from his chair after the display and in the next scene he has with Lucrezia, he explicitely says that they all but had sex again.
